I, the undersigned, am the parent or legal guardian of the above-named minor ("Participant"). I hereby acknowledge and agree to the following:

Assumption of Risk:I understand that participation in mobile arcade activities involves certain inherent risks, including but not limited to exposure to flashing lights, loud sounds, physical activity, use of video gaming equipment, low ceiling heights, stadium seating and interaction with other participants.

Release of Liability:I release and hold harmless Arcade Shark Inc., its owners, employees, contractors, affiliates, and representatives from any claims, liabilities, demands, or actions arising out of or related to any injury, loss, or damage to person or property that may occur during the event, including travel to and from the mobile arcade.

Medical Authorization:In the event of an emergency, I authorize Arcade Shark Inc. staff to seek medical attention for my child and agree to be financially responsible for any resulting costs.

Behaviour Agreement:I understand that inappropriate or unsafe behaviour may result in removal from the mobile arcade without refund.

COVID-19 & Health Safety:I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk of exposure or infection. I will not send my child if they are exhibiting symptoms or have been in contact with a confirmed case.
